New Patient Registration

We are pleased to welcome new patients.  Please register at reception and bring with you your NHS number and proof of address.  If you have arrived from overseas, bring your passport/visa too. 

Provided you live within our practice boundary, we will carry out an instant health check and you will be able to see a doctor two days later.  Patients who are aged over 45 will be given an appointment with a healthcare assistant for a more in-depth new patient check.  Please read our patient agreement.
